In order to ensure the accurate identification of students who may speak a language other than English,
the Newark Public Schools is surveying all public school students upon registration. Your assistance in
completing the survey form on the reverse side now and returning it immediately to the school staff will
be greatly appreciated. The District is required by the New Jersey Department of Education and the
federal Office of Civil Rights to receive a response for each child including those who speak English as
their sole or primary language. Respectfully,
