IV. POTRDILO O ZDRAVSTVENEM STANJU / INFORMAÇÕES SANITÁRIAS / HEALTH INFORMATION
Podpisani uradni veterinar s tem potrjujem, da so bile omenjene živali pregledane v obdobju desetih dni
pred izvozom in da je bilo s kliničnim pregledom ugotovljeno, da so zdrave, brez zunanjih zajedavcev ter
sposobne za prevoz. /
O veterinário oficial que abaixo subscreve declara que os animais identificados foram examinados dentro
dos dez dias anteriores ao embarque e estavam clinicamente sadios, isentos de ectoparasitas à inspeção
clínica e aptos para o transporte. /
I, the undersigned official veterinarian herby certify that the above-mentioned animals were examined
within 10 days prior to export and found healthy after clinical examination, free from ectoparasites and fit to
travel.
1. V primeru prvega cepljenja proti steklini mačk in psov, starejših od treh mesecev, je bilo cepljenje opravljeno vsaj 30
dni pred vstopom živali. Veljavnost cepljenja je eno leto. Cepljenje se ne zahteva za živali, ki se izvažajo iz držav, prostih
stekline. / A vacinação anti-rábica foi aplicada em caninos e felinos maiores de três meses de idade e foi realizada, pelo
menos, trinta dias antes da data de ingresso do animal, no caso de primeira vacinação e com validade de um ano. Não
aplicável para animais exportados de países livres de raiva. / In case of first anti-rabies vaccination of cats and dogs over
three months of age, the vaccination was conducted at least 30 days prior to the date of entry of the animal, with
vaccination validity of one year. This requirement shall not apply to animals exported from rabies-free countries.
2. Opisana žival je bila pregledana v obdobju desetih dni pred izvozom in ni kazala nobenih kliničnih znakov bolezni,
prenosljivih prek te živalske vrste. / O animal identificado foi examinado nos dez dias anteriores ao embarque, não
apresentando nenhum sinal clínico de doenças próprias da espécie. / The above-mentioned animal was examined within
ten days of export and did not show any clinical signs of diseases transmissible by this species.

1. V primeru živali iz držav, na ozemlju katerih je pri OIE uradno objavljena prisotnost konjske kuge in/ali mrzlice doline
Rift, mora spričevalo potrjevati tudi naslednje podatke: / No caso de animais provenientes de países que declaram
oficialmente junto ao OIE a presença em seu território de Peste Eqüina Africana e/ou Febre do Vale do Rift, no
certificado deverão constar também as seguintes informações: / In case of animals coming from countries, on whose
territory the presence of African horse sickness and/or Rift Valley fever has officially been declared by the OIE, the
certificate shall certify also the following information:
a) da v zadnjih treh (3) letih v kraju izvora in v krogu s polmerom petdesetih (50) kilometrov okoli kraja izvora ni bilo
registriranih primerov bolezni iz prvega odstavka teh opomb; / que no lugar de origem e num raio de cinqüenta (50)
quilômetros deste, não foram registrados casos das doenças citadas no "caput" do presente inciso, nos últimos três
(3) anos; / that in the preceding three (3) years in the place of origin and in the radius of fifty (50) kilometres around
the place of origin there have been no registered cases of diseases referred to in paragraph 1 of these notes;
b) da v tem obdobju živali niso bivale v območjih s prisotnostjo omenjenih bolezni. / que os animais não estiveram,
durante este período, em regiões afetadas por estas doenças. / that in this period the animals were not kept in
regions affected by the presence of these diseases.
2. Karantena po uvozu se ne zahteva za živali, ki izpolnjujejo zgoraj omenjene zahteve. V primeru suma prisotnosti
infekcijskih ali kužnih bolezni, zoonoz ali bolezni z visokim tveganjem določi pristojni veterinarski organ ukrepe za
izolacijo živali ter uvede ustrezne zdravstvene ukrepe. / Os animais que cumprirem com os requisitos anteriores não
realizarão quarentena de importação. Em caso de suspeita de doenças infecto-contagiosas, zoonóticas ou de alto
risco, a autoridade veterinária determinará as providências que assegurem seu isolamento e correspondentes
medidas sanitárias. / Animals complying with the above requirements shall not be subject to quarantine on
importation. In case of suspected presence of an infectious or contagious disease, zoonosis or high risk disease, the
veterinary authority shall provide for the isolation of the animals and institute pertaining sanitary measures.
